COVID vaccinations are available at gp on beaufort.
Prior to booking your appointment please check that you are eligible for the vaccine by going onto the Health Department website at https://covid-vaccine.healthdirect.gov.au/eligibility
Bookings will be via our online booking system HOTDOC through our website or the app, or by phoning the surgery on 08 9262 8600.
We are committed in providing a safe, quality COVID 19 vaccination service to all.

Reminder prior to vaccination
- It remains Mandatory to wear a Mask at all times whilst at the practice.
- Check-in using the Safe WA app
- Complete all forms sent to you via HotDoc or staff
- No vaccines will be administered if you are unwell
- Please DO NOT attend the practice if you are displaying COVID-19 symptoms

If you have COVID / flu like symptoms – cough, sore throat, runny nose, temperature, loss of smell, loss of taste or gastro symptoms, please arrange a telephone or video consult with your doctor.
Even though you may be fully vaccinated this is not a 100% guarantee that you will not contract COVID-19 or spread to others. So help us to protect vulnerable patients and staff.
If you have any type of respiratory symptoms, please do not attend the practice. Book a telehealth / video health appointment to see your regular doctor.

Our Story
“Addiscombe House” is a landmark federation style building located at 691 Beaufort Street, Mount Lawley. Built in 1906 as a private residence, the building was purchased by Doctor Goran Pervan in 2005 then gutted and renovated to restore its heritage features and house a contemporary, fully equippped medical practice, gp on beaufort. In 2008, Dr Pervan was awarded the City of Stirling Heritage Award for the sensitive adaptation of a heritage building.

Entrance Mural
The mural painted on the hallway is by local Western Australian street artist Jerome Davenport aka Ketones6000.
The mural is comprised of three elements, the historical portrait, the Rose Mallee representing new life and the flower of life pattern tying it all together.
Back Wall Mural
This Mural was painted by sign painter Mick Haines. The blue tree is a symbol of The Blue Tree Project which helps to raise awareness and encourage people to speak up about mental wellbeing.
Whilst the words RUOK? refer to the importance of asking the question and checking-in with our community. Both initiatives are significant to us at gp on beaufort and we hope this Mural starts the conversation in you.
